Thank you for submitting your candidacy form. We will be emailing you in the next 24-48 hours with detailed information (allow extra time if your form was submitted during the weekend), but here are a few things to expect:

Required: Recent MRI and MRI Report Submission

In order to evaluate your condition, we require a recent MRI (within the last two years) and the associated MRI report. This can be delivered to us by mail or uploaded electronically. Instructions for this will be provided in the introduction email.

Required: Review Fee and Phone Review with Doctor

You will be scheduled for a phone call with one of our doctors to discuss your MRI and your condition. Plan on approximately 20-minutes for this call. We charge a $200 fee for this review, which is applied to the cost of the procedure if you are a candidate and decide to proceed.

Decision: Electronic or Mail Submission of Materials

We offer our patients two distinct ways to submit their information, but we can work with you if you prefer to use a combination of electronic and mail / phone submission of your materials and payment. Instructions will be provided in the email we will send you shortly.

Electronic Submission – You will complete a short, secure online medical history form and provide a credit card for the $200 review fee and 20-minute doctor phone call. You will zip up your MRI files and MRI report and upload them electronically. However, you may choose to mail a disc if you prefer.

Regular Mail Submission – You have the option of completing the secure Medical History Form online or you can download it here and fill it out. You may pay the $200 fee for the 20-minute doctor review phone call with a credit card by phone, or you can mail a check with your other materials. Additional information:

The Regenexx Family of Procedures can range from $1200 – $6000 and are not covered by insurance. Please understand that you will be required to travel to the Denver, CO area for treatment. Travel and accommodations are not included in this fee, but we do have discount agreements with hotels in the area – Click here for details on visiting and staying in the Denver area.

If undergoing the Regenexx-SD Procedure, your first visit entails the bone marrow, blood draw and examination (for one 2-hour appointment). Your re-injection process is the same or next day. Depending on your condition and the treatment options, multiple trips may be required.
